Social anxiety is rough, I know, trust me, I know, same with depression. But you just have to push through it. I know that doesn't sound like sound or helpful advice, but its the only way. Purposefully do things that aren't comfortable.
Example: I used to use the self-checkout things at grocery stores. I stopped doing that, because I realized that, for me, it wasn't about convenience, it was about feeding my social anxiety by not engaging others.

Start working out. Take up a hobby that will urge discipline. Get some friends that will help back you up during the hard times, and there will be hard times.
You have to do it man, there's no other way, and you don't want to be a NEET forever. Seek help from your parents, or siblings, if you've got them - just be honest with them, and ask for help. Maybe find a good shrink, or just someone who'll listen to you talk - sometimes, that really helps.

Do things that will help boost your confidence. Working out will help. Taking up a hobby, learning a skill, will help too.

Something that helped me was starting to take morning walks - its not that hard, doesn't require much, and if you do it right, you'll be too worn out to stay up all night dicking around in the internet or what have you. Getting on a normative sleep cycle can do wonders. Work on your nutrition too - you're probably eatting dog shit, so stop doing that and get ahold of easy-to-prepare food (because you're a lazy faggot, I know) that's actually good for you.

If you want to go really extreme? Cut your internet access at home. Just cancel it. You can always go someplace with wifi or something if you want to shitpost for a few hours, but that will keep you from being able to just sit at the fucking computer like a fat stupid NEET with no friends all day.
