Tips; I think Crickhollow (shire) is a village in Wales called Crickhowell. But Tolkien may have just used the name, not the place. I think Fornost is now known as Barrow-in-Furness, in the Lake District of England. Using those two places as references you might be able to scale and orientate an overlay map correctly. The Shire was supposed to be on the same latitude as Oxford, though it's bigger (many hundreds of miles across) than it seems in the films. Also, remember "North" may have been quite a bit off the modern direction ~12,000 years ago.

The hills marked in Tolkien's maps of the shire/ western middle earth roughly line up with the hills/mountains of Ireland and Wales. The main problem I found when comparing these locations was that it's difficult to discern exactly what counted as "mountains" in the Middle earth maps.

It all falls apart at the misty mountains unfortunately. They're supposed to be directly East of the Shire (so Oxford) and extend hundreds of miles in a straight line north and south. The only mountain range I can see in the rough location is the Norwegian/Swedish mountains, which are wildly off what they should be if Tolkien's maps were based on real locations millennia ago. Unless the Misty mountains were the Urals, in which case the scale of the whole of Middle earth gets a LOT bigger.

I've been researching some more based on your effort. You were close, the misty mountains range further south, though. (And the distances on the Middle Earth map aren't reliable so it skews everything a bit). As a guide, Mount Gram (in Middle Earth) is Ben Nevis in Scotland.

Isengard is basically where Cardiff is today, at the end of the range of Mountains that make up the "Backbone" of Britain (The Welsh part is the Cambrian range if you want to look at it on Google Earth). The gap of Rohan and the fords of the Isen are where the Severn estuary is today, around the area between Bristol and Cardiff (Severn=Isen).

South of the estuary are the Quantock and Mendip hills (Ered Nimrais). Helms Deep is on a Northern spur of the Ered Nimrais, i.e. the northern tip of the Mendip Hills, near Bristol.
