Death Squads - and closely related means of extrajudicial execution - have been used in many ways.
The Death Squads of 1980s El Salvador were very different from the death squads currently operated by Duterte in 2016 Philippines.
Death squads can be run by drug lords, human traffickers, paramilitaries, formal militaries, espionage shops, police departments, etc.
Duterte has linked his death squad purge to specific weapons policy: he wants small arms and handcuffs to repress soft civilian targets. He does not want jet fighters or rockets to hit military targets. This humble policy makes him much less useful to the USA and much more focused on his own backyard than is usual.
Not all death squad leaders are so humble. Many want both power to repress dissidents within their borders, and power to project force outside their borders.
The 21st century will probably see many different kinds of death squads. In many cases, the English-speaking world will not know of these death squads. There are probably death squads in Saudi Arabia - but no one will ever report about them, because they will kill any journalist that gets close enough to discover them!
Duterte has killed a small number of people - just a few thousand. He will never get up to Pol Pot levels of body count at that rate.
But oddly, Duterte has inspired great obedience. Drug users are turning themselves in out of fear.
This might be the dominant paradigm of the 21st century. Death squads might be used to make people obedient, not to encourage massive class warfare, ethnic cleansing, etc.
21st century has seen the decentralization of media and information exchange. we've seen armies move from mass tactics towards highly mobile, smaller units. there's no reason to assume that law enforcement won't follow this trend

Clearly death squads CAN be effective.
The 21st century may well be characterized by an efflorescence of death squads. But there is no reason to believe that all of them will be right wing. Many will be street criminals with no political sophistication.
Memeing the right wing is one thing; memeing competition between all possible factions is different.
However, One important polarity is statist versus anti-statist. Some death squads will be prostate and will seek to promote centralized power. But pro-state authoritarianism is not necessarily left or right. In Duterte's case, it is right-wing. But in the case of the Vincent Foster assassination, it was probably left-wing.

The death squads in Philippines are state sanctioned. Death squads there enjoy the police feeding them information and overlooking their activities. The only thing they have to worry about is criminals finding out who they are or getting killed during a hit.
In developed countries death squads aren't able to enjoy the same sanctions. The state will come down on you with every resource available to it. Working with more people becomes a liability. If one of them get caught you all get caught and it only takes the weakest link to talk. A one man or two man squad with someone you trust your life with is only doable.
You'd have to meticulously watch your op sec. One mistake that leads back to you and you're done. And there's thousands of different mistakes you could make without even knowing it.
