First of all, the right hand is supposed to reach to the bottom three pads, and the left hand the top three. For that matter, by top three, I do not mean the buttons that are placed fucking impossibly towards the top, but instead the three that the right hand is wrong placed upon.
One of the most annoying things, however, is the mouthpiece. It looks like the mouthpiece fits inside the 'head' of the saxophone (which is usually detachable on any sane model). The entire connection is basically just a cork. It's very similar to a clarinet in that way.
Speaking of which, the bottom part of the saxophone near the end is a wee bit bloated.

thing I have learned a long time ago: when you think you're almost done with the image, flip it horizontally, and a fuckton of problems will become visible. Fix them all, then flip again. Repeat until satisfied.
Then preferably save the image, don't look at it for a week, and THEN check if it still looks good
the thing is, as you make the picture, your mind gets used to how it looks, and starts accepting it as "normal", disregarding errors

As for the actual eye level, since the face is pretty much completely straight, just have the eye corners line up with each other. vertically. How high up they should be specifically is your call.

the change is not in the flipping, the change is in your mind. Again, you slowly get used to the not-so-good looking things as you draw them. It mostly comes from the fact that you have seen the previous, even-worse looking stages, so improvement over those outweighs the objective shortcomings in your mind.
I say the face is decent, and the most off-model thing is the bangs, which are too flat and off to the side.
As for the eyes, you can expose it more as you edit the bangs.
