It's not the metadata so much as it is the hash derived from the image.
Every file whether or not it's an image file (jpg, png, gif, etc) video file (mkv, mp4, avi, etc) text file or any other kind of file, algorithms were designed to take the data from those files down to their very bit structure and generate a "hash" from it, which is a unique string of characters (numbers and letters typically) that is unique only to that file, or specific bit pattern. Even changing a single bit (1 or 0) in the file changes the hash entirely.
Modern computers and network infrastructure are incredibly robust nowadays and are able to preserve and check file and data integrity quite well. So when you download a file from the internet, whether deliberate in involuntarily such as in a cached thumbnail, the bit structure is exactly the same as from the source you downloaded it from meaning that if the same algorithm is ran on both files from the source and your copy that you downloaded, they'd generate the same hash (independent hashes for the original image and thumbnail).
Now what does all this have to do with your question?
Well, companies like Microsoft and Google have developed technologies that scan your directory structures of network drives or even local drives if you allow it, and generates hashes from the files it scans and detects on the fly whilst simultaneously comparing the generated hash to a known hash stored on their servers of a confirmed file. Law enforcement agencies utilize these technologies quite often.
Additionally digital forensics analysts do the same thing, except locally. They immediately hash every file they deem as evidence and will sometimes reference the taken hashes to known hashes of files.
So basically if there's a copy of a CP image on your computer that has been unmodified to a version they've hashed already, they'll know it's the same file. Whether or not they determine if it was downloaded deliberately or with malicious intent, well that's up to human judgement.
