When the current champ calls "new champ", the first poster to claim it is our champ. The first six legal Pokemon suggested once champ has been claimed or "new team" has been called will be the team our champ uses to battle. Suggesters are not to make multiple suggestions for a team unless 15 minutes have passed since their last request, or if the champ specifically asks for re-requests. The champ will be battling in OU unless he specifies otherwise.

Once you get above 1500 elo, teams start to have strange Pokemon on them, usually as niche picks or counters. Shit like Specially Defensive Leftovers Camerupt, because it's a hazard setter that can switch into all variants of Magearna. It's also why you find Smogon OU sets for Rhyperior and Gourgeist and some other bottom tier mons: Anti-meta picks that punish overoptimized teams.

Tier/Format refers to usage. Rank refers to how well a Pokemon performs in his own tier.
For some time, Dhelmise was in UU despite being near-universally agreed to be a terrible Pokemon by most competitive players. Dhelmise is a 'noob trap' Pokemon. At first glace he appears to be a decent Pokemon. He had good offensive stats, three stab moves, access to rapid spin, decent defensive stats, and is (I think) the only rapid spinner that can actively prevent the opponent from rapid spinning. However, this doesn't take into account things like other viable Pokemon (Decidueye and Trevenant offensively, Gourgeist defensively) or whether that niche is viable (because the niche for a bulky offensive spinner that can spinblock is extremely niche). He's similar to Donphan in that respect (or Electivire/Magmortar in the lower tiers).
It got to a point where the UU forums actively banned discussion of Dhelmise and made a special D rank in the viability rankings (in brown and puke green colours, no less) specifically for him.
Worth mentioning is that it is possible for a Pokemon to be in a low tier/format (Rhyperior, Cofagrigus, Quagsire) but still have a decent rank. Quagsire, for example, is PU by usage but C+ rank in OU viability.
