The objective of the survey is to determine some aspects that make a video game "good" in the eyes of gamers.
Any help is certainly appreciated. If you have any comments about the form itself, please let me know as well.

Your survey sucks, it does not distinguish between what we want in a game or what we realistically think will make a game popular. For example, I think for game to be an esport it should have balanced characters or none at all, but reality has proved that it does not need that to become popular.

The whole point of the assignment is to make a Survey, not to actually find out what makes a game good.
So basically they picked a topic that is specifically difficult to determine objectively. The challenge is to formulate the questions in the best way to get the most meaningful data.

Okay, then you should probably learn that your data is meaningless unless you go at it from a scope far beyond just games. Economics is also a part of it (All game companies have been suffering since 2007), culture (Numerous consoles have failed in Japan, but succeeded in the West, and vice versa), marketing (There's a reason the Wii was a success beyond "Filthy casuals"), and the basic understanding of human nature (PDF related).
Also, you should come to realize that all people do 'NOT have good tastes what so ever. None of us do. None of us ever have, and none of us ever will.
