The US took Guantanamo Bay as a naval base after the Spanish-American war to help it enforce the Monroe Doctrine across the Caribbean and South America. In return, they pay rent to the Cuban government for the land. The US has continued to send checks to the Cuban government even after the revolution, but the commies have only cashed a single one. The Cubans are unable to take back the base without the US just invading them and taking over their island, so they've just let it slide for the most part. Also because it gives them commie points so they can kvetch about the imperial-aggressor US govt having part of their land. The US hangs onto it because it's still a good naval base, plus they can offload anyone they don't like into its prison without actually putting them on US soil.
