I think it's a little of both, and to a certain extent can actually be learned or unlearned, after all Haidt himself was by his own admission a big progressive lefty when he began his research into moral psychology, and now is much more centrist, even slightly conservative in his orientation because he spent a significant amount of time not only looking into the morals of others, but also looking at the morals he held so dearly, and realized the blind spots inherent in leftist thinking. You either have to be very curious, so much so that you are willing to challenge your own belief system, or something either very traumatic has to happen to you or something happens that you can't easily square with your current political beliefs.

Care/harm is one of the most basic moral foundations humans have, very much related to the fight or flight response. I should probably rephrase "equality" as "fairness" since it more accurately describes this moral dimension. You are somewhat correct in that they do tend to conceptualize the world negatively, but the main reason for that is because these types of people often engage in more utopian thinking, which often means that they think the world SHOULD be more "fair" and "equal", and with respect to the care/harm dimension that nobody should experience anything discomforting, which further explains why many of these people are women since women often score much higher in these two dimensions than men do. Of course reality doesn't simply conform to how yours or anybody else's wishes, often going against their utopic thinking, which causes depression and negative thinking.
