Does Steam ban R rated shit? Physical stores will obviously never carry it but online sales are your best bet and, like it or not, Steam is the only way to really make big money. You can otherwise sell the game from your own site easily.

Retailers are more than happy to sell R/M rated shit. It's when the product is rated AO or X that retailers get pissy. Unless it was taken off, Steam does allow Hatred to be sold, but I don't know about games rated AO due to sexual content.

The porn part is interesting since there are a few VNs that actually bypass it. "rooftop spirits" or something was a recently released VN with no censoring whatsoever. Perhaps as long as Porn isn't the main focus of the game, it's allowed there?
The "inapropriate or offensive content" however… This is double silly since it's both one of those situations where "if you have to ask…" but also just who exactly is getting offended here? It's entertainment and it can be quarantined to it's own corner so regular users don't see it. Age checks are a thing, even online, and as long as they aren't advertized in the front page, everyone that will ever actually look at those games is looking for something similar, therefore the only people being offended would be the retards actually looking to be offended.

Blame retailers and platform holders. The AO rating exists, and the ESRB is willing to use it. It shouldn't be a black mark–it should be just another rating label. But big retailers generally won't stock AO games, and console manufacturers won't allow AO games on their platforms.

Teen and Mature ratings are actually starting to flip flop, with T games featuring boobs and guts, and M games being censored for triggering content. Personally I preferred the era of MS-DOS games where you had to play through a game to see if it was okay for kids.

Take a look at movies like 300 or Deadpool. The executives wanted them to be PG so the whole family could enjoy them (i.e.: more cash flowing if your mom and granny can watch them too). Their success without sacrifying the mature content has perplexed them and now they are slowly daring to do more risky stuff (I believe they say some profanity in Suicide Squad).
Now, a teen might easily have the cash to buy a AAA movie ticket, but it's not that easy to them to have the cash for buying a AAA videogame. And many parents won't gladly buy them an AO game to their teeens. So….
