Medicanon opinion: This is bullshit. The guy was attempting a pericardial aspiration - to remove blood from the cavity between the heart and its membrane bag, to treat a condition called 'pericardial tamponade where blood builds up and collapses the heart chambers. It is a last-ditch intervention where the child is in cardiorespiratory arrest following suspected blunt force trauma to the chest, not gas poisoning. The water is in the syringe so the operator can detect the puff of blood which tells them that the tip of the needle is in the right place to aspirate.
They would have had a better chance putting a breating tube in and ventolating the kid, but it looks like they only had kit suitable for adults.

Also, I'm not seeing evidence of other interventions. The video quality is pretty poor, but it looks like some tubing is just laid across the legs. I don't see actual IV access. I don't know of anyone who would try to intubate (Mac on the table) without pushing a sedative. If that was the goal or was unsuccessful, why not go for the cricothyroidotomy?
