Depends on the file format but usually, you can just paste the data (view in in a binary or hex editor) of one file after the other file into one "document". Some file formats/programs complain if they find all that junk data at the end or beginning so it really only works for stuff like this.

Here I've pictured the text where it is overlapped in a way that would make a text recognition program unable to make the changes you've suggested it has, as well as a second picture where the low quality of the scanned image has tricked the function into thinking the text colour is lighter than the ink would have been.

The third picture shows the original letter "K" in the pictured "Kansas" is obscured in a strange way at its head. Initially I accounted for this by reasoning that this was a result of the quality of the scan but if that were the case the whole letter would have a consistently bad quality, perhaps not something to worry about but it's a strange effect I noticed didn't match with the rest of the document.

All this would beg the question, why, when under scrutiny for the whereabouts of your birth, why would you scan such a document with the mentioned features enabled and at such a low quality?

I still personally question the validity of the document. Be it due to forgery or, as you said, due to the lack of forethought by the individuals who scanned and distributed the document to create a copy that is undoubtedly legitimate. If it turns out to be the former (which would be proved by a redistribution of a higher quality document) then it would make me question the ability of the individuals and their affiliated groups to do things correctly the first time.
