If you care about the poor people in Appalachia and you feel like nobody else does, maybe it's because there are people living in various levels of poverty all over America and it keep most people occupied. Or, if Appalachia is just worse than literally any part of the USA, maybe it's because you're too lazy to do the work required to raise awareness and help out.

They should care about both groups. But pretending the poor whites would have been okay if it wasn't for the Syrians is dishonest.
We have a similar issue in the UK with "health tourism" which costs the NHS £200m per year.
The public have been convinced this is a big issue even though the NHS is underfunded by £15-20bn per year.
It distracts people from the fact that the government is deliberately running the NHS down so they can privatise it by manipulating anti foreigner sentiment.
It also ignores the fact that foreigners could legally be required to hold private health insurance and that immigrant labour contributes £8bn per year in tax.

Both parties have had plenty of time to help poor people in Appalachia, neither did it so playing the blame game doesn't work.

How are we supposed to help our own people if we can't help ourselves, etc? How can we devote resources to one problem if we aren't devoting enough resources to another problem?

The refugee crisis exists regardless of USA action. This is something that people don't seem to have a grip on. There's more to it than just "let them in" or "don't let them in," because they still are traveling long distances and not all of them are going to the same place. There is a whole load of humanitarian concerns, and as long as there are people in the west with the means to assist it's useless to try and tell them to stop caring.
