I need more games where enemies throw a bazillion projectiles at you which you have to dodge from a first-person perspective. No hitscan bullshit. Is it me, or has a real first-person shmup not been tried yet? There are some attempts here and there but they're not really the main focus of the game, I bet someone could make it work if they tried.

Serious Sam is more about throwing a whole bunch of enemies at you than it is about dodging shit. Though a lot of enemies do throw projectiles at you, they're not really the main focus of avoiding damage.
I kind of understand that, but most dodgeable projectiles in Doom are simple fireballs and the levels often let you avoid fireballs by retreating behind a corner. Compare this to Marathon where you have enemies which fire homing projectiles, long-range spraying fireballs, medium-range grenades, and so on. Hence why I posted it in the OP.

Shmups pretty much require 360° awareness, so to fit that in a FPS you have to scale everything down so that you can realistically understand what's happening around you at all times without needing to see it or resort to shitty tricks like scaling down aggressiveness of enemies you don't have LoS on, scaling fov way up might help but even then it starts becoming way too confusing past a certain fov even with more specific solutions that allow for good 140°+ fov. If you want a practical example Assault Android Cactus has a FPS mode you can unlock.

Easy to fix that with level design. Instead of an open field, with enemies all around, imagine only being attacked by one end of a wide hallway. Most shmups are more similar to Galaga than they are to Asteroids, so a shmup fps shouldn't try to mess with that.

I remember how nuDoom had enemies running all around you, but sometimes you'd get blindsided because you couldn't anticipate an attack from behind because you couldn't hear it coming at all. One thing first-person games definitely should focus on is sound when it comes to having to know what is going on around you, like in Devil Daggers. For example, enemies outside of your line of sight could telegraph their attack with a clear positional sound which indicates what direction the attack is coming from, but level design which largely moves in one-direction seems to be a more suitable solution.

Resident Evil 4 made enemies offscreen less likely to attack you. While this can be exploited on high levels of play, the mentality of having the most important threats onscreen is valuable.

But if you can see everything that is hostile in your LoS you have to resort to non aimed pattern a lot more because streaming aimed burst is much easier in FPS than it is in top down view, and from my experience understanding that kind of bullet patterns in FPS view is really hard so it also come back to having to scale it down, homing attacks are also another thing you have to approach differently between top down and FPS, there's also the "attack from above" style of attacks you can't do as well in FPS. And then there's the whole problem of whether you wanna have the player stick to 2D movement or let him have more or less limited 3D movement (which goes anywhere from single jump and crouching to ToG -tier ridiculousness where you're basically zooming around all over the place especially with that one negative recoil gun)

The problem with that is you'd be looking more often at the radar than the screen when you're surrounded.
That's a good one, I do fondly remember boosting through a narrow passageway leading to an open room with enemies waiting to ambush me at the end of that passageway behind the exit, just so I could fly through and do an 180 as I destroyed them without taking damage like Max Payne in zero gravity. Descent also had a neat approach where enemy AI factored in how you'd dodge their attacks, they weren't simply stationary objects which spammed bullets at you, and the level design made endless circlestrafing an impossibility, on top of the enemy AI being able to lead its shots as well.

Kleers are faster than you but you can dodge them due to how they attack but you can't deal with much more than 4-6 being in close range unless you're lucky or really fucking good at visually representing what happens around you without seeing it. Bulls also are way faster than you but have a lot of inertia and acceleration time so you can also dodge them up to a point.
Both are better dealt with via superior firepower at long ranges though, but the games do throw a couple of close range encounters you can't avoid with those two enemies.

Some Doom mods get pretty close to bullet hell. Reelism comes to mind, but there are far more deliberate attempts to recreate bullet hell. Touhou Doom is one… if you like Touhou. But there were better ones I remember playing on Skulltag.

Drunken Robot Pornography is what you're looking for OP. The level at 14 minutes is a great example. Footage is not of the final game, but it's pretty much spot-on: lotsa enemies with different actions, your motion is constrained by the level design but flexible thanks to your jetpack, shots shots & more shots. The game is funky, I don't recall the dubstep music this video features so if it stayed in the final version, it mustn't have been obnoxious.
