No it doesn't, the game has all the content in it already and just checks that a file with a specific name exists. If the file that can contain literally nothing as long as the name is correct is present then it uses uncensored mode.

Not really. There are several games hosted on steam that have extractable models that can be naked but those aren't accessible without extracting them. By hosting those games they technically host those nude models but you can't access them without messing with the game files. Adding a file to the folder, while not as significant as extracting models and the like, is still changing the game files.

My friends can already see I played 2110 hours of TF2, I have no dignity left to lose on Steam and I suspect most people can say the same.
